have type 1(insulin dependent, usually childhood onset) diabetes?

OR
If you are
aged between 1 and 20, do any of the following members of your family
have type 1(insulin dependent, usually childhood onset) diabetes?
- Your mother
or father
- Your brother
or sister
- Your son
or daughter
- Your grandmother
or grandfather
- Your aunt
or uncle
- Your niece
or nephew
- Your half-brother
or half-sister
- Your first
cousin (son or daughter of an aunt or uncle)
